Things to be aware of prior to your visit

If your child needs pediatric cancer care or treatment for a blood disorder, we ask that you come prepared to your appointment to maximize your visit. Please take time in advance to do each of the following:

  • Call our office for location and parking details if you’re uncertain
  • Compile your health insurance card, your child's Social Security number, their parents' names and your co-payment amount
  • Fill your child in on what they should expect during their visit
  • Review and notate your child's complete medical history
  • Write down any questions you or your child wish to ask

If this will be your child’s first visit with us, please arrive 30 minutes early to fill out required paperwork. You can also download, print and complete the forms beforehand. We also ask that you bring some materials with you on the day of your appointment, including your insurance card and photo ID, a full list of the current medications your child is taking and copies of any recent X-rays and lab results.

Please call our clinic at least 24 hours in advance if you need to cancel or reschedule an existing appointment. While we understand that emergencies happen, our team makes every effort to stay on schedule.
