How to schedule an appointment

So that you and your child can be seen in a timely manner by the right specialist for them, a physician's referral is required. Additional authorization from your insurance company may also be required, so please check with them before your appointment. If required, please contact your primary care physician before your appointment so they can provide your authorization.


What to bring to your appointment

To make your visit run as smoothly as possible, please bring the following:

  • Current medications
  • Insurance card and co-payment
  • Records from your child's visits to the primary care physician, emergency room and other specialists, as well as previous hospitalizations or surgeries related to their condition
  • Referral from your primary care physician
  • Relevant test results, including computerized tomography (CT) scans, kidney ultrasounds, voiding cystourethrography (VCUG) scans, renal scans, as well as blood tests and urine tests

When to arrive for your appointment

If this is your first visit, please arrive 30 minutes before your appointment time to complete paperwork. If you download the required forms and complete them before the appointment, please arrive 15 minutes early so we can verify your information and collect the co-payment. We strongly recommend that you fill out the forms beforehand so your visit with the doctor can be as productive as possible.

For follow-up visits, please arrive 15 minutes early to allow enough time to verify information changes, as well as collect your co-payment.


What to expect during your visit

You will always meet with a doctor when you come to our facility, and for your first appointment, we schedule extra time so your child can have the maximum benefit. During this visit, you and your child will have about 45 to 60 minutes with the doctor.
